Child Enrollment Application

Thank you for considering enrolling your child (or children) into the Northwest Montana Head Start program. We look forward to working with your child and your family!

Eligibility

Child must be 3 years old

Child must NOT turn 5 before September 10th

Your completed application packet must include:

Income Verification–For each adult in the household that has a legal relationship to the child or to the child’s primary caregiver.

Immunization Records–Please contact us if you need any assistance in obtaining documentation.
